Healthcare Spending Account Eligible: All consults can be paid for with a healthcare spending account, or flex spending account.
If you are booking an appointment for someone else, please put the MOTHER'S name, address, phone, & email in the appropriate fields so we can contact her directly about her appointment. You can put "booked by......" in the notes section to tell us your name.
Lactation Link understands that plans can change, especially when you have children (we’ve got ‘em too)! If you find that you need to cancel or reschedule your consultation, just give us a 12-hour heads up via email prior to your scheduled consultation appointment time. If you request to cancel or reschedule after this time or do not keep your appointment as scheduled, we are unable to process any refund of payment and no superbill for potential insurance reimbursement can be given.
